Pay Rate: $18.00 – $24.00 / hour

Covenant Case Management Services (CCMS) is seeking a motivated, detail-oriented Associate Professional (AP) with a passion for intellectual and developmental disabilities (I/DD) to support our residential service lines. This includes Supported Living, Alternative Family Living (AFL), and the Long-Term Care Support (LTCS) framework.

This entry-level role works in close partnership with a Qualified Professional (QP) under the strategic oversight of the Director of Residential. Designed as a growth pathway into a broader QP role with clearly defined milestones and supervision, you will expand caseload oversight, ensure seamless coordination across treatment teams, and uphold a high-quality "white-glove" experience for our families.

What You Will Do | Responsibilities

Residential Onboarding & Family Coordination

  • Smooth Transitions: Collaborate with Legally Responsible Parties (LRP), treatment teams, intake, and onboarding staff to initiate services for members entering AFL or Supported Living.

  • White-Glove Service: Serve as the primary operational touchpoint during residential placement orientation, ensuring a welcoming family experience and a seamless handoff to the assigned QP.

  • Program Delivery: Develop and deliver onboarding programs for families and residential providers to establish clear roles and communication channels from day one.

  • Care Alignment: Connect with care managers to confirm accurate Individual Support Plan (ISP) start dates and align service initiation with physical housing move-in timelines.

Service Coordination & Tracking

  • Daily Scheduling: Implement and manage daily schedules of residential services and living activities in collaboration with the QP and site leadership.

  • Team Communication: Maintain ongoing communication with care managers, DSPs, and home managers to align daily service delivery with the ISP and NC Innovations Waiver requirements.

  • Skill Integration: Coordinate with the QP to integrate skill-building interventions (independent living skills, community integration) into members' daily routines.

Data Collection, Documentation & Audit Readiness

  • Data Management: Collect, organize, and enter residential data (daily shift notes, progress updates, ISP data points, behavior tracking) into internal systems.

  • Audit-Ready Records: Assist in completing residential service notes under supervision to ensure timely, accurate records matching NC DMH/DD/SAS standards.

  • Compliance Monitoring: Monitor field-level service adherence across LTCS, Supported Living, and AFL lines. Verify home-level certifications (CPR, First Aid, state modules) and audit routine safety drill records.

Assessments & Clinical Support

  • Assessments: Support the preparation and execution of residential environment, safety, and functional assessments as directed by the QP.

  • Information Gathering: Collect details from residential team members, families, and housing providers to inform assessments and ongoing ISP updates.

Crisis & Escalation Support

  • Protocol Development: Assist in developing residential crisis protocols and behavioral support integrations.

  • First-Line Escalation: Serve as an operational escalation point for residential and family concerns when the QP or clinical staff are temporarily unavailable.

Cross-Functional & Culture Support

  • Teamwork: Provide backup support to colleagues during peak periods, vacations, or absences to ensure operational continuity.

  • Culture & Ethics: Maintain high standards of data integrity, confidentiality (HIPAA), and ethics. Engage constructively with teammates to foster a positive, growth-oriented culture.

What We Are Looking For | Qualifications

  • Experience: Entry-level welcome; 0–2 years of experience in human services, I/DD support, or residential/AFL/Supportive Living environments preferred.

  • Core Skills: Strong organizational, communication, and teamwork abilities; high attention to detail; and readiness to develop rapidly under clinical supervision.

  • Tech & Resources: Reliable personal transportation for site visits, a smartphone, and dependable internet access.

  • Physical Demands: Ability to assist with lifting up to 50 pounds as needed for client care tasks in a residential setting.

Education & Licensure Requirements

To qualify as an Associate Professional (AP), candidates must possess a qualifying degree/credential matching one of the following state-regulated categories:

  • Graduate Level (Human Services): Master’s degree with under one (1) year of professional, post-graduate human services experience.

  • Undergraduate Level (Human Services): Bachelor’s degree in a relevant human services field paired with fewer than two (2) years of full-time, post-degree experience.

  • Undergraduate Level (Alternative Fields): Bachelor’s degree in a non-human services discipline paired with under four (4) years of post-bachelor's professional human services experience.

  • Registered Nursing (NC Licensure): Active NC RN license with less than four (4) years of experience supporting MH/DD/SA populations.

Note: Associate Professionals function under the immediate clinical guidance of a departmental Qualified Professional (QP) and must maintain a current, signed, annual supervision roadmap.
